Are You Insured by Blue Cross Blue Shield (BCBS of NC or Federal Employee ) or Aetna Plans?

Do you want to go forward as a self-pay patient? If so, you will receive a Superbill receipt that you can send to your health insurance provider so they can reimburse your out-of-pocket cost ($140/hour; $70/half hour)

I acknowledge that there is a fee of $35 if I cancel my appointment with less than 24 hours notice. I acknowledge that I must read & sign the Informed Consent & Release of Liability form before my appointment can proceed.
